Lean Six-Sigma survey

I am a doctoral student at the Manchester Business School, currently studying the application of Lean Six Sigma in various organizations. As part of my reseach, i have prepared the following survey in order to investigate the similarities/differences of the two approaches, level of integration and project selection.
